A Salt Lake City native, Dr. Jared Fairbanks graduated with his Doctor of Osteopathic Medicine from Touro University Nevada, College of Osteopathic Medicine. He completed a Family Medicine Residency at Kingman Regional Medical Center in Kingman, Arizona, where he served as the Chief Resident. Dr. Fairbanks began his career as a primary care physician and wound care and hyperbaric medicine specialist. He enjoys working with diabetic patients to help them make positive life changes. He is board-certified by the American Board of Osteopathic Family Physicians.

Johana grew up in El Salvador, and as an international student, she completed her nursing degree at the University of Nevada, Las Vegas. As a nurse, Johana recognized some issues that needed to be addressed at the primary care level. She went on to earn her Doctor in Nursing Practice from the same institution in 2022. She is excited to be a part of Intermountain Health’s culture of care for both its patients and associates. Johana is fluent in Spanish. In another life, Johana was meant to be a veterinarian. She owns six dogs and a 150-pound African tortoise.
